Paperless Forms:

Paperless forms are provided to all new patients to complete prior to their scheduled appointment. Please CLICK HERE to fill out On-Line forms. They can be done conveniently on any phone, tablet or computer.

Patient Screening:

COVID-19 screening is completed on the phone prior to appointments and upon entering the office.

Temperature Check:

Using a contact-free thermometer, we check all patients, staff and accompanying persons.

Surgically Clean Air Filtration System:

The JADE air purification system utilizes Surgically Clean Air's multi-stage air purifying technologies that work together to clean, purify and re-energize the indoor air. The multi stage filtration system captures dust particles, pollen, volatile organic chemicals (VOCs), mold, allergens, viruses, odors (like formaldehyde and smoke), bio-aerosols, nitrous oxide, and many other pollutants that are found in the inside air in workplaces and residences. The negative ion generator makes indoor feel more pure, clean and energized to help combat fatigue

Social Distancing:

In order to maintain social distancing guidelines, we have a limit of one patient in the clinic at a time. Patients are required to follow their scheduled appointment times. Walk-ins are given a card with our information and instructed to call for any questions or booking of appointments.

Plexiglass shield:

Plexiglass barriers are installed to minimize the contact with front desk staff.

Hand Sanitizer & Masks:

Everyone entering the clinic is required to wear a face mask and use an alcohol based hand sanitizer. We supply both.

Accompanied Person:

If a patient requires a substitute decision maker or cargvier to be present during the appointment we allowing one person at the time of appointment.
